1. For membership application to be processed, it must be accompanied, in all categories by a one-time non-refundable fee of $250.00. In addition, a refundable first year annual due of $500.00 for organizational affiliate, $750.00 for honorary/individual membership and $1,000.00 for corporate membership is required.
2. Limited Liability companies (LLC) must attached a copy of the organization's Articles of Incorporation showing the state of filing and the appropriate state agency. Any adopted operating agreement must also be attached. All amendments to the above documents must also be attached
3. Unincorporated organizations must attach a copy of the articles of association or any similar, dated organizing document with, at least two signatories. All dated and signed amendment must also be attached
4. A TRUST must include a trust and dated copy of the TRUST agreement. All dated and signed amendment must also be attached
5. A current signed and adopted bylaws must be attached showing the date of the adoption. If no bylaws exist, please explain how the organization's officers, directors and trustees are selected
6. List the names, titles and mailing addresses of all the current officers, directors and trustees.
7. Every information requested in this membership application form must be truthfully and completely supplied. WIC reserves the right to reject any membership application if it deems it fit to do so. Please refer to WIC's constitution and bylaws for more information
